This form is confidential. You may remain fully anonymous. Reports are reviewed personally and used to build documented advocacy reports for the Texas Legislature. You will never be identified without your explicit permission.

Please note: I do not hold any position of authority within TDCJ or any governmental agency, and I am unable to intervene in individual cases. My purpose is to listen, receive concerns that are voluntarily shared, and advocate for greater awareness and meaningful reform where possible.
